What to Bring

Please be sure to bring the following items to your appointment:

1. Insurance card
2. List of current medications
3. Copies of any records related to your office visit
4. Completed initial patient information forms
5. Any films or studies related to your present problem and their reports; please make sure to bring copies of the films or CDs to the visit.
6. If needed, a friend or family member to help you ask questions
